Your enrollment checklist

  1. Download and complete the correct agreement:
  2. Complete the Release of Information (owner operators) or Random Driver list (employers).
  3. Complete the payment sheet.
  4. Fax everything to (626) 813-1088.

After payment is received

Once payment is received, an official Certificate of Enrollment and a Consortium Member Packet will be mailed to you. A Certificate of Enrollment can be faxed to you upon request. Your certificate proves participation in a DOT-compliant random drug and alcohol program for CHP/BIT inspections and DOT audits.
