Consortium Enrollment

To enroll in a D.O.T.-approved consortium, follow the steps below — an LWD representative will contact you the same or next business day.

If you are an Owner Operator

  1. Fill out the online form below and submit
  2. Download and complete the Owner/Operator Agreement (PDF)
  3. Complete the Release of Information
  4. Complete the payment sheet
  5. Fax everything to (626) 813-1088

If you are an Employer

  1. Fill out the online form below and submit
  2. Download and complete the Employer Agreement (PDF)
  3. Complete the Random Driver list
  4. Complete the payment sheet
  5. Fax everything to (626) 813-1088

What happens next

Upon clicking "Enroll Now" and faxing the appropriate agreement above, an LWD representative will contact you the same day or the following business day to confirm your enrollment and collect payment. Once payment is received, an official Certificate of Enrollment and a Consortium Member Packet will be mailed to you. A Certificate of Enrollment can be faxed to you upon request.
