More Locations — Collection Site Search
Over 2,000 collection sites are available for you to choose from. Each center handles drug and alcohol collections and processes your CCF or alcohol testing form with our designated DOT-approved laboratories.
Important: you must obtain a Custody and Control Form (CCF) from LWD prior to arriving at the collection center.
If you cannot send your driver to LWD's primary collection facility or arrange for an on-site visit, choose one of the many sites using the search steps below.
- Obtain a Custody and Control Form from LWD. Call (626) 813-1086 or request one online before arriving at the collection center. LWD mails the authorized form to you at no charge. See a sample CCF (opens an image).
- Search for an LWD/Quest-approved collection center. Use the locator on the Quest Diagnostics website: enter your ZIP code, enter a radius in miles, and select the drug/alcohol screening and urine drug screen options — then click Search. Open the Quest Diagnostics locator.
- Schedule your test. After you have chosen a collection site, contact them directly to schedule an appointment — most sites offer walk-in service. Remember to take your LWD CCF form with you. Questions? Call (626) 813-1086.
